New Aesthetic Client Intake Form
For aesthetic appointments including skin rejuvenation, laser treatments, injectables, skin analysis, body contouring, skincare, and cosmetic treatment consultations.
Medical New Patient Form — Male
For male medical visits including hormone concerns, testosterone therapy, TRT, weight management, wellness care, medications, medical history, and required treatment consents.
Medical New Patient Form — Female
For female medical visits including hormone concerns, HRT, weight management, wellness care, pregnancy/nursing screening, medications, medical history, and required treatment consents.
What to expect when you arrive.
When you arrive on the day of your first appointment, you will check in at the front desk. One of our staff members will provide you with any paperwork you were unable to complete before your visit. Depending on your concerns, you may meet with a Medical Assistant, Nutritional Counselor, or our aesthetic staff.
To help make your appointment smoother, please download and complete the form that best matches your visit type. Aesthetic patients should complete the Aesthetic Intake Form. Medical patients should complete the Medical Male or Medical Female form.
